Navigator Alzheimer’s at Home in New Canaan Program Expands Eligibility

Waveny LifeCare Network announces that the Navigator Alzheimer’s at Home in New Canaan Program, housed on Waveny’s 3 Farm Road campus, is expanding its eligibility to include individuals and/or their loved ones with any dementia diagnosis living in New Canaan. Going forward the program will be referred to as the Navigator Alzheimer’s and Dementia at Home for New Canaan program (ADAH).

“We are pleased to expand the program to include more families struggling with any type of dementia in New Canaan,” said Executive Director of Navigation, Kathleen Benevento. “We are confident this change will provide more family caregivers with access to vital support services that are so greatly needed.”

The Navigator program has been meticulously tailored to deliver a concierge-level service, entirely free to the New Canaan community, to address the evolving and unforeseen needs that arise when caring for a loved one with Alzheimer’s or dementia. The Navigator team are specialists in Alzheimer’s, dementia, care options and long-term planning, providing 24/7/365 assistance including medical support, community resources, activities, long term care planning, emotional support and many other needs.  Members of the Navigator ADAH Program also receive priority access to the entire Waveny LifeCare Network healthcare continuum. If Waveny is not able to assist with your request, referrals are made to a trusted Waveny partner.

The program currently has two staff members, Executive Director of Navigation Kathleen Benevento and Navigator Chris DiElsi, LMSW. If you would like to connect with a Navigator for support services,  please contact Chris DiElsi at [email protected][email protected], or (203) 594-5495 (direct) or via the Navigator cell phone at (203) 594-5289, answered 24/7/365.
